Community Health Worker - Court Compliance - HHSJob Description PURPOSE SUMMARY. Under the supervision of the designated Program Administrator in the Court Compliance Division, this position provides community support services, client services and care navigation services to people involved in or at risk of becoming involved in the criminal justice system. ADDITIONAL DUTIES. Other duties may be assigned. Position requires, on occasion, work beyond normal business hours and/or extended hours, nights and weekends. QUALIFICATIONS. A. Education. High school diploma or GED. B. Experience. Four years full time experience in health care, health promotion, outreach, community, and customer service activities or case management. Bilingual (English/Spanish) is required. C. Education/Experience substitution. In accordance with County policy. D. Licenses/Certifications. Valid driver’s license. Must maintain a satisfactory driving record in accordance with County policy. Certified as a Peer Support Specialist, Community Health Worker or Family Peer Support Specialist is preferred. E. Other (e.g., post-offer medical exam, polygraph, background check, driver’s license record, etc.). Must pass background and driver’s license check. KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED BY THE POSITION: Must have knowledge of: Peer support and recovery-oriented support; healthcare and social services systems; motivational interviewing techniques; community-based education principles; and general knowledge of community resources. Must have skills to: Operate general office and computer equipment; be proficient in computer software to include Microsoft Office Suite; maintain accurate records; and communicate effectively in verbal and written form with others. Must have ability to: Work independently; observe clients, assess client’s demeanor and living conditions, recognize behavior changes, and take effective action; prioritize and multi-task; problem solve; interact with a wide range of people; develop collaborative partnerships; utilize referral networks within the local health care system; and maintain confidentiality; communicate effectively both orally and in writing.
